"By approximately 1245, Mindaugas was being called supreme king, and it was understood that all of the Lithuanian lands and those of certain neighbours – southern Selonia, Black Rus’, possibly the Rus’ian castles of Breslav and Minsk were in the hands of a single sovereign."

Yes, but understood by whom?

The Rus, at the time comprised of tribes, and in 1245, they were still basically under feudal bondage to the Mongols.

The Mongols had terrorised the Kievan Rus (or Rus as they were known at the time) and the latter had brought the Mongols off, and even contracted Mongols to fight for them.

But, around 1245, Mindaugas was still engaged in "on again-off again" battles with other aspirants to the Crown, which itself was bestowed by the Pope, not the people. 

Lithuania, as a region, had been in existance for centuries with only local lords in control. Mindaugas unified Lithuania by deals and by defeating some of the opposition, but he wasn't truly recognised  as the first King until about the 19th Century.
